Bilingual Case Manager (Spanish/English), Developmental Disabilities Program

PURPOSE & SCOPE: The Case manager is a member of the CFCS Developmental Disabilities Program team, and assists families, children, youth and adult family members referred by the Department of Developmental Services (DDS) in developing annual Family Support Plans and assist in accessing resources needed to obtain goals. The purpose of the DD program is to:
• Promote ability of families referred by the Department of Development Services to support their members in strengthening life skills, peer connections, parent connections and self-care, and positive family life;
• Promote disabled individuals in achieving life skills, education and employment goals;
• Develop and utilize resources to support families caring for disabled members;
• Provide a range of services, including case management, assessment, service planning, referrals, crisis intervention and guidance in using available resources to assist families in meeting service goals.
The case manager is supervised by the Director of Developmental Disabilities Programs, and works directly with families, children and youth in their homes and community.
DUTIES AND RESPONSIBILITIES: The Case Manager:
• Conducts one-on-one in-home or community based visits with clients and/or families as required, based on family need or contract requirements to monitor effectiveness of support plan;
• Assists families in accessing resources needed identified in the Family Support Plan;
• Collaborates with other CFCS staff in developing and coordinating family activities and programs;
• Assists in recruitment, assessment and evaluation of respite care workers and skilled trainers for identified families.
• Conducts and prepares comprehensive assessments of families
Duties and responsibilities are carried out in conformance with relevant Competencies (attached).
REQUIRED EDUCATION & EXPERIENCE:
• Bachelor’s degree in social work, psychology, education or a related field; 3 years’ experience working with families, children and adults with developmental disabilities may be substituted for the Bachelor’s degree.
• Professional licensure where applicable;
• Minimum of 1 years’ experience in working with families, youth and adults with developmental disabilities.
Other requirements:
• Valid driver’s license and properly insured vehicle
• Clearance of background checks as required by state and federal law.
• Bi-lingual preferred, verbally and written. We are seeking candidates that speak and write English and Spanish (for interpretation and translation.)
